Welcome to Angels in Your HomeSince 1997, Angels in Your Home has provided a full range of home care services in Rochester, NY, and surrounding communities. Our agency was created in response to the need for home care services in Monroe County and surrounding counties.
Personal Care Assistance (PCA)The PCA program is a Medicaid program that covers home care services that are typically considered “non-skilled”, however a certified aide provides services to the client. Aides can provide personal care such as toileting, showering, dressing, errands, grooming and light house-keeping (activities of daily living). However, they cannot administer medications or assist with other treatments prescribed by physicians. PCAs are supervised by a Registered Nurse, who ensures that the plan of care as determined by county nurses is followed, and personal care needs of the client are met. In order to qualify for this service, you need to be a Medicaid recipient, a referral must be made to your county’s Department of Human Services.
Nursing Home Transition and Diversion Waiver (NHTD)The NHTD Waiver is a Medicaid program designed to assist individuals with disabilities and seniors that are at risk of going into a nursing home, or for people that live in a nursing home and want to live in the community. This Waiver is structured around each person’s strengths, needs, choices and goals. Each individual makes decisions around their services and care, which leads to greater fulfillment and greater independence. The unique services offered through this Waiver allow people to take control of their lives, work with service providers to ensure they can successfully live in the community and avoid nursing home placement.
Home and Community Support Service (HCSS)HCSS is a discrete service available to participants of the NHTD Waiver when oversight and supervision is needed to help the participant maintain their health and welfare. Oversight and supervision is often delivered in the way of prompting, cuing, directing, and suggesting. HCSS staff can also provide personal care to participants. Furthermore, HCSS can be used to assist participant to accomplish their daily activities, such as light housekeeping and activities of daily living. HCSS can be used to provide personal care needs such as bathing, toileting and dressing.
Traumatic Brain Injury Waiver (TBI)The TBI Waiver is a Medicaid program designed to assist individuals with traumatic brain injuries who want to live in their community and require support to maintain independence. The TBI Waiver is structured around each person’s strengths, needs, choices and goals. Each individual makes decisions around their services and care, which leads to greater fulfillment and independence. The unique services offered through this Waiver allow people to take control of their lives and work with service providers to ensure they can successfully live in the community and avoid nursing home placement.
