The Consumer Directed Personal Assistance Program is a New York State Medicaid plan that allows individuals to hire family members, friends, or a Personal Assistant (PA) of their choice to provide assistance with activities of daily living (ADL). Personal Care Aides assist with activities of daily living, such as bathing, grooming, dressing, feeding, shopping, light housekeeping, and medication reminders, helping individuals maintain their personal care routines.
Home Health Aides offer personal care and support, including daily living activities like bathing, grooming, dressing, feeding, shopping, and light housekeeping. They can also assist with medications, take vital signs, perform range-of-motion exercises, and prepare and serve meals.
Our licensed nurses deliver clinical care to patients in the home or community setting. They provide personalized medical care for surgery recovery, infusion treatments, chronic conditions, wound care, medication regimens, and other specialized services, while following doctor's orders.
These services offer support to children and adults with long-term needs, allowing them to remain in their homes and communities. Waiver services are designed for individuals who would otherwise require the level of care provided in a long-term care facility.
